Enrollment/Registration
Registration can only take place at the Central Office at Westchester, 758 Scott Boulevard, between the hours of 8-11 am and 1-4 pm.
When registering students new to City Schools of Decatur the following documentation is required:
- certified birth certificate
- social security card
- Georgia state immunization form (3231)
- Georgia state hearing, vision and dental screening form (3300)
- verification of residency (see chart below)

Mortgage/Deed/Lease (student's name must be on housing authority lease) Payment/Coupon Book Property Tax Settlement Statement

A check or a driver ís license is not adequate proof of residence. Verification of residency is not required for tuition students.
All new to district registration must be approved by the City Schools of Decatur District Offices first, then the following documentation for the Registrar at Decatur High School:
Registration Checklist
- Sealed withdrawal package
- Birth Certificate
- Immunization Record-Form 3231
- Vision/Hearing/Dental-Form 3300
- Social Security card
- School Transcript
- Attendance record
- Discipline Record
- Once a student is fully enrolled at the CSD Distict office, that student then goes to the high school for scheduling classes.
- As the counselors may already have appointments, it is best for families to schedule a conference by contacting the school's Registrar (404-370-4420 ext. 116).
- Counselors will need copies of official transcripts or current report cards in order to assist with accurate placement.
